Dear Mr. Rivera,
O&L General Contractors, Inc. appreciates the opportunity to work with Roseland Community Hospital. We are pleased to submit the following proposal outlining the scope of work, materials, and terms for the painting and plaster services requested.
| SCOPE OF WORK |
- O&L will supply all painting and plaster materials required to complete the work.
- O&L will provide all labor, tools, and equipment necessary to perform and complete the project.
- Work areas will be fully protected with plastic sheeting on floors and walls throughout the duration of the project.
- O&L will furnish all materials and labor required to execute and finish the job to completion.
- The hospital’s engineer/facilities manager will provide the required paint codes and brand specifications prior to the start of work.
- Paint used for this project will be Sherwin-Williams.
| WORK SCHEDULE |
- Standard working hours: 8:00 AM – 5:00 PM, Monday through Friday.
- In the event of an emergency outside of standard hours or on weekends, O&L will be on standby and available to work at the same contracted rate.

- W-9 Form
- Certificate of Insurance
- Signed Proposal
- Valid Contractor License
